The Concordia College athletic training staff welcomes you to the athletic training area of the Concordia College athletics web site. This area will have information for both current and perspective student-athletes as well as parents.

NEW STUDENT-ATHLETES
If you are an incoming freshman or transfer student-athlete, you will be MAILED a packet containing the medical documents needed to participate in intercollegiate athletics at Concordia College (NY). These documents are more detailed and include vaccination paperwork. The vaccination paperwork only needs to be completed by incoming freshmen and transfer students.
RETURNING STUDENT-ATHLETES
If you are a sophomore through graduate student-athlete, you must fill out and print these forms EACH year.
1) Pre-Participation Physical Form (must be filled out by a physician or nurse practitioner) 2) Emergency Contact Form 3) Acknowledgement of Insurance Requirements/Medical Consent & Assumption of Risk/HIPPA FormPlease mail the completed documents listed above by August 1st to:

This form is filled out on your computer, saved and then e-mailed directly to Felicia Chianese at Felicia.chainese@concordia-ny.edu
Lastly, the Department of Athletics at Concordia College (NY) has made it mandatory that ALL student-athletes be tested for Sickle Cell Trait. This test needs only be performed once during the student-athlete’s time at Concordia College (NY). Mail in the results to the College’s address listed above. The Health Services Office at Concordia College (NY) offers the Sickle Cell Trait Test for $10, however, you may get the test done on your own.
